Question: Is it known whether it is smoke (i.e. 2.5SM) that is the pathogenic actor in cookfire emmisions or the CO (carbon monoxide)?

Try two weeks of cooking all your food over an open fire made of sticks, grass & $hit in a hut, then come back & tell us how your lungs & eyes feel & how succulent & nutritious the food was; that’s a credible study.
given the total population dependent on indoor cooking 4 million seems to me a rather low percentage of total. Many more die of other causes such as random violence, disease, lack of hygiene, undernourishment, traffic etc. The least of their worries seems to me to be cooking on wood
